Arterial Conduits for Surgical Revascularization

چکیده

Coronary artery bypass grafting (CABG) remains a commonly performed procedure for obstructive coronary artery disease that is not treatable with either medical therapy or percutaneous intervention. It has been well studied and documented that using the left internal thoracic artery (LITA) reduces morbidity and mortality compared with using saphenous vein grafts (SVGs) when performing a CABG.1 One reason for the observed improved outcomes is the anatomic structure and the preserved and functional endothelium of the internal thoracic artery compared with an SVG. It was assumed that this inherent superiority of the LITA would also be true for the right internal thoracic artery (RITA) and other arterial conduits that could be safely procured and used for bypassing coronary arteries. However, this assumed inherent superiority of using any arterial conduit compared with using an SVG to targets other than the left anterior descending artery has been much harder to prove. Two recent reports of using a meta-analysis of radial artery versus SVG reached different conclusions.

Off - pump coronary artery bypass with multiple arterial conduits

Objective: to analyze intra-hospital results in patients undergoing elective off-pump total arterial revascularization and identify morbidity and mortality predictors using this surgical strategy. Methods: From May 1999 to February 2004, 203 myocardial revascularization procedures with total arterial revascularization without cardiopulmonary bypass (off-pump) were carried out in patients with m...

Conduit options in coronary artery bypass surgery.

The choice of graft conduit is crucial to the success of coronary artery bypass grafting (CABG) because the patency of a coronary conduit is closely associated with an uneventful postoperative course and a better long-term patient survival. The standard conduits used for CABG are the greater saphenous vein (GSV) and the internal thoracic artery (ITA). An excellent substitute conduit for coronar...

Comparison of the right internal thoracic artery and radial artery as a second arterial conduit in ‘Y’ composite fashion in patients undergoing coronary artery bypass grafting using total arterial revascularization

Introduction: The use of two arterial conduits for CABG is rapidly increasing. The second arterial conduit to LITA is usually RITA or radial artery. We sought to compare outcomes when either RITA or radial artery is exclusively used as a Y composite graft to LITA for total arterial revascularization. Material and methods:  We retrospectively analyzed 231 ...

Arterial grafting and complete revascularization: challenge or compromise?

PURPOSE OF REVIEW Arterial grafting is superior to venous grafting in coronary artery bypass graft surgery with respect to graft patency and long-term patient outcome, but it may be difficult to achieve complete arterial revascularization. RECENT FINDINGS Use of arterial grafts, especially bilateral internal mammary artery grafts, is not common, whereas there are clear indications that it may...

Surgical infrainguinal revascularization for peripheral arterial disease: factors affecting patency rate

Background: Peripheral arterial disease is a source of morbidity and mortality. Surgical vascular reconstruction is a treatment option but probability of failure and complications are important concerns. In this study, we evaluated outcome of surgical infrainguinal reconstruction and factors affecting graft patency for a period of one year. Methods: In this cohort study, 85 consecutive patie...

Current trends in surgical revascularization of multivessel coronary artery disease with arterial grafts.

It is well known that graft patency determines prognosis in coronary artery bypass grafting. Numerous reports over the past 20 years have documented superior patency and prognosis when multiple arterial grafts are used. The use of the left internal thoracic artery to graft the left anterior descending artery has been widely accepted as the gold standard for surgical treatment of coronary diseas...
